COPD Explained: Early Warning Signs You Should Never Ignore

Chronic Obstructive Pulmonary Disease (COPD) is a progressive condition characterised by persistent respiratory symptoms and airflow limitation due to airway and alveolar abnormalities. It is the third leading cause of death globally, yet it remains vastly under-diagnosed — largely because patients attribute their early symptoms to just getting older or smoker's cough.

The earliest warning signs deserve serious attention. A persistent productive cough, especially prominent in the mornings, is often the first signal. Increasing breathlessness during activities that were previously effortless — climbing a flight of stairs, walking briskly, or carrying groceries — indicates progressive airflow obstruction. Frequent respiratory infections that take longer than usual to resolve, and audible wheezing that your family notices before you do, are equally significant red flags.

Spirometry performed after administration of a bronchodilator is the definitive diagnostic test. A post-bronchodilator FEV₁/FVC ratio below 0.70 confirms the diagnosis. Crucially, spirometry can detect significant airflow obstruction years — sometimes a decade — before symptoms become functionally limiting, making opportunistic screening of current and ex-smokers over the age of 35 extremely valuable.

Smoking cessation remains the single most powerful intervention at any stage of COPD. It is the only treatment proven to slow the progressive decline in lung function. Pharmacotherapy with nicotine replacement, varenicline, or bupropion, combined with structured behavioural support, achieves the highest long-term quit rates.

Modern bronchodilator therapy — particularly the combination of long-acting β₂-agonists (LABAs) with long-acting muscarinic antagonists (LAMAs) — has transformed quality of life for COPD patients across all disease stages. Pulmonary rehabilitation, encompassing supervised exercise training and education, produces improvements in exercise tolerance and health-related quality of life that no medication alone can match.
